First of all, are you Dutch? Cause if you are, you probably do not need a work permit to work in UK.
In their website, they have mentioned about how qualifications are assessed if you are unable to find your institution:

"If you cannot find your academic qualifications on the points-based calculator, you may still wish to claim points for it. If so, you should contact UK NARIC directly for an assessment of the level of the qualification and, if UK NARIC confirms it is of the required level, obtain a letter and/or confirmation certificate from UK NARIC. You can find a link to the UK NARIC website on the right of this page. It is an independent website, for which we are not responsible. It may charge a fee for confirming qualifications.
If UK NARIC cannot confirm the equivalence of your qualification, we will not award points for it. In this case, you may wish to use an alternative qualification if you have one."

You can try to look up their website and see if your institution is on their list, or otherwise you need to contact them regarding to this, but they might charge. May I ask what course did you take and institution? If it is technical course, then it is usually accepted.

I only submitted my degree certificate without the result of individual exams, so I think it is absolutely fine. Of cause, if the marks are impressive and you have them on your hand, might as well send them along as it would do no harm. I did not provide any proof for earlier education(high school), as it doesn't seem to be the most relevant.

My bank statement had my full name, however, I think 'K Lau' should also be acceptable as use of initial is quite common in UK. I am sure you can request the bank statement again with the full name on from your bank, or request a letter from your bank stating that 'K Lau' is you.
